Field Trip Request Form

Due to changes in district transportation routes this year, only two district buses are available for most trips. Trips requiring a third bus will need to have the additional transportation bid out, so please plan and submit requests well in advance.

• Field trip requests must be submitted at least 30 days in advance to allow for the approval process.
• District buses must return by 2:00 PM at the absolute latest.
• Last-minute requests may not be able to be accommodated.
• The new field trip form includes an option to track the approval process. Staff are encouraged to enable this feature to confirm that their building administrator has forwarded the request for Board approval.
